Step 1:  Schedule Your Free Consultation

Your Goal: Ensure we are the right fit for your Post-Rehab or Medical-Exercise needs.

We offer a complimentary, no-obligation 15-minute consultation. This is a quick opportunity to discuss your goals, review your history, and make sure our specialized approach aligns with your expectations.

Step 2:  After Completing Your Consult

Fill out paperwork (approx 15 minutes) to help us get to know You and personalize your full program:

  1. Health Questionnaire

  2. MET-Q

  3. Goals

For post-rehab clients:

  1. Please fill out your appropriate form (as directed by Sheryl) under our Functional Assessments

  2. Please sign both forms: release of information and liability

After your paperwork is complete, you will schedule your Initial In-Person Assessment (60 minutes). This is where we measure your current strength, flexibility, and mobility to build your starting program. Click here to schedule: in-person assessment


Step 3:  Program Launch & Support

Now it's time to start!

  • Schedule Sessions: Use our calendly link below to book your 4 workout sessions (typically one per week) and your Re-Assessment (45 mins). Note: A 24-hour cancellation notice is required.

  • Trainerize Setup: Check your email for your Trainerize invite. Please watch the short tutorial videos: https://www.living4health.com/trainerizevideos to familiarize yourself with the app we use for your independent workouts.

  • Your First Session: Address and parking details will be provided in your scheduling confirmation email. Feel free to arrive a few minutes early to warm up on the cardio machine.
